Tuition is due monthly by check on the first Tuesday of the month that school is in session. In order to keep costs lower for families, processing fees will be added for any families that pay online.

Child Care Subsidy and Scholarship Applications
The Mayapple School is an approved Child Care Subsidy Vendor. This program is administered by the Virginia Department of Social Services and helps eligible lower-income families pay for child care. To find out if you qualify and to apply, please visit Child Care VA.

We take efforts to provide as much confidentiality as possible for families who apply for or participate in child care subsidy and scholarship programs, including privacy of identity.
The Mayapple School is also a 501(c)3 non-profit, and as part of our mission, we fundraise for and give scholarships yearly to families. We encourage families to apply who:
have income at or below 220% of the Federal Povery Limit or have additional extenuating financial circumstances that cause usuable income to be at or below this level.
AND/OR have a Mayapple School student with special needs that require additional staff to support those needs.
have also applied for Child Care Subsidy. Scholarships may be awarded regardless of subsidy award.

New Student Forms
Read the Parent Handbook.
Fill out the Parent Handbook Agreement.
Have your child’s PCP fill out the School Entrance Health Form.
Complete the Child Registration Form fully - do not leave any blanks.
Fill out the Ongoing Permission Slip for Price Park Field Trips.
Decide on Media Permissions for your child.
For full day students only: fill out the Sunscreen & Bug Spray Form (further directions are inside the parent handbook).

Staff Forms
Read the Parent Handbook. (before employment)
Provide copy of college diploma (if applicable) or high school diploma. (before employment)
Provide copy of resume with previous relevant experience. (before employment)
Provide copy of driver’s license or other photo and age identification. (before employment)
Complete Virginia Preservice Training for Child Care Staff - 10-hour course (within 30 days of employment)
Virginia Annual Health and Safety Update Training for Child Care Staff - 3-hour course (yearly, except first year of employment).
Complete 16 total hours of professional development. Various options qualify. Professional Development Note Taking Template (yearly)
Provide proof of Tuberculosis Screening or Testing. Save receipts for reimbursement up to $35. (within 30 days of employment; every 2 years thereafter)
Can be completed at local health departments with appointment.
Can be completed at local urgent care facilities (not covered by insurance).
Complete the following tax forms (before employment)
Complete CHLD 2016 - Health Screening for Childcare Professionals. (within 30 days of employment)
Complete CWSE 5691 - Recognizing & Reporting Child Abuse & Neglect (For Educators). (before employment)
Complete a qualified Pediatric First Aid & CPR course from American Heart Association or American Red Cross (must include an in person component). (within 30 days of employment)

Paid classes are available through Virginia Tech Recreational Sports.
Complete in-person facility-specific training - Staff Orientation Documentation Form. (before employment)
Complete Sworn Statement of Affirmation. (before employment)
Schedule and complete fingerprinting appointment using these Fingerprinting Directions. (before employment)
Complete OBI form for Child Protective Services background check (link will be sent to your email during the hiring process). (before employment)
Out of State Background Checks - If you have lived in any other state in the past 5 years, you will need to get more background checks: criminal background; sex offender registry; child abuse and neglect searches for each state, as according to this chart. (before employment)
